The Initial Breastfeeding Care Visit is designed for infants of any age, and can be used to address primarily maternal medical or lactation concerns as well. This visit includes two components:

  1. A Breastfeeding Medicine Consultation for mother and baby, either in-person or via telemedicine in the comfort of your home. Visits range from 60-120 minutes in length, depending on complexity, scheduled directly with Dr. Renno at a time that is convenient for you.

  2. Seven days of support via telephone and text, beginning the day after the appointment.

Please schedule a free Meet and Greet with Dr. Renno to discuss your eligibility for Telemedicine care (which is determined entirely by medical complexity) prior to registration. Maternal Concerns

  • Milk Production: Delayed lactogenesis, low milk supply, hyperlactation/oversupply.

  • Painful Feeding: Nipple damage, plugged ducts, milk blebs, dermatitis, vasospasm.

  • Infections: Mastitis, thrush, mammary dysbiosis.

  • Maternal Wellbeing: Debilitating fatigue, postpartum depression, dysphoric milk ejection reflex.

  • Anatomic Barriers: Flat or inverted nipples, insufficient glandular tissue, history of breast surgery/injury.

  • Medical Barriers: Maternal thyroid disease, PCOS, medication contraindications.

  • Complex Nursing Scenarios: Nursing multiples, nursing in the NICU, tandem nursing, inducing lactation for adoption.

  • Pumping: Exclusive pumping, painful pumping, returning to work, milk storage.

  • Weaning: Abrupt weaning due to medical emergencies, difficulty weaning when desired.

Infant Concerns

  • Growth: Neonatal weight loss, slow weight gain, failure to thrive.

  • Jaundice: Will work closely with your pediatrician to accurately diagnose and manage.

  • Latch: Painful, shallow, inefficient, losing suction, choking, nipple shield dependence.

  • Tongue Tie: Frenotomy during the visit if indicated, non-invasive management options.

  • Medical Complications: GERD, cow milk protein intolerance, food allergies.

  • Challenging Cases: Cleft lip and palate, Trisomy 21, prematurity, hypotonia, anatomic barriers.

  • Infant Refusal: Bottle preference, breast refusal, bottle refusal.


Follow-up Breastfeeding Care for mother and baby can be arranged after this initial consultation, if needed.
